We’re seeking a Social Worker (MSW) to support the social, emotional, and practical needs of our residents and their families — developing care plans, providing counseling, and serving as a strong advocate for their well-being.
Key Responsibilities:
- Conduct psychosocial assessments and develop individualized care plans
- Provide individual, group, and family counseling
- Advocate for residents’ dignity, rights, and appropriate care
- Foster communication between residents, families, and care teams
- Coordinate discharge planning and connect families with community resources
- Provide crisis intervention and end-of-life support
- Educate care teams on culturally competent care
- Maintain accurate documentation and collaborate in care team meetings
- Monitor care plans’ effectiveness and contribute to quality improvement
The Ideal Candidate:
- Master’s degree in Social Work (MSW)
- Experience in health care or assisted living preferred
- Strong interpersonal and communication skills
- Collaborative team player who advocates for patient dignity
