We’re seeking an MDS Coordinator to help develop individualized care plans and maximize reimbursement — all while upholding the highest standards of care.
Key Responsibilities:
- Develop care plans based on MDS assessments in collaboration with the interdisciplinary team
- Ensure the facility is compliant with all regulations and guidelines related to MDS
- Communicate effectively with staff, families, and healthcare providers to coordinate care
- Maximize reimbursement by accurately reflecting care needs in the MDS
- Update MDS assessments regularly to reflect health or condition changes
- Contribute to overall quality assurance by assuring completeness and accuracy of MDS documents
- Gather information through direct observation and collaboration with care team members
- Licensed Registered Nurse (RN) required
- RAC-CT (Resident Assessment Coordinator - Certified) certification required
- Experience with MDS and care plan development
- Strong communication and organizational skills
- Detail-oriented and comfortable navigating regulations
- Collaborative team player who advocates for resident care
- Familiar with reimbursement processes and compliance requirements
