What You’ll Do:
As a Clinical Triage Nurse, you'll be responsible for evaluating patient concerns by phone, coordinating follow-up care, and working closely with providers and office staff to promote timely, patient-centered care and positive outcomes.
The Right Candidate:
- Active CT LPN license.
- Active BLS Certification.
- 3+ years’ experience as an LPN, preferably in a clinical triage or outpatient setting.
- Prior clinical phone triage experience required.
- Experience with EMR systems required.
- Strong understanding of clinical workflows and patient communication.
